FreeMedForms Portable is an open-source software application designed specifically for helping doctors keep a database with medical records. The utility can be run on Windows, Linux, Mac OS X, and FreeBSD.

FreeMedForms Portable lets you store information about patients. The patient information can be edited. It is saved to XML file format so you can alter it according to your preferences. Plus, you may add several subforms to your main patient file.
The built-in scheduler allows you to create an appointment for your patients. You can preview all appointments via a calendar.
Thanks to the PMHx (Patient Medical History) component, you can automate several tasks like identifying inappropriate medications for a certain patient. The prescription module is a nifty feature that allows you to access several treatment databases (France, Canada, US, and Belgium).
